1. Filled in application form with the attested true copies of the concerned marks cards and other certificates as required, should be submitted to the Principal, Santushti College of Medical & Higher Studies, Mirzapur., within the last date specified. Incomplete applications will be rejected.
2. Application form for admission to first year B.A.M.S. course can be obtained on payment of Rs. 500/- by cash or D.D. drawn in favour of Director, Santushti College of Medical & Higher Studies, Mirzapur.
3. The application fee paid will not be refunded. The submission of duly filled application is not a guarantee for admission. Admission is decided on merit and performance in the interview.
4. Selected candidates will be called for interview by the Principal and should present themselves before the selection committee, accompanied by parent or guardian at their own cost.
5. Candidate appearing before the selection committee should bring with him/her original documents cited below without fail.
6. S.S.C. /Xth Marks Card showing the date of birth
7. b. 12th /Equivalent Examination I & II year Marks Card.
8. Character & Conduct certificate from Institute last studied.
9. Scheduled Caste / Tribe certificate (Validity Certificate from concerned authority to be enclosed).
10. Twelve colour passport size and six stamp size recent photographs.
11. Transfer or migration certificate wherever applicable.
12. Eligibility certificate from Mahatma Gandhi Kashi Vidyapeeth, Varanasi.
13. Any other certificates or records required. The list of selected candidates will be notified on the College notice board. Intimation will be sent by post. The selected candidates shall pay all the fees/charges within the specified date, failing which the seat will be allotted to next eligible candidate.
14. Fees once paid will not be refunded or adjusted to other accounts under any circumstances.
15. Candidate once admitted shall be bound by the prevailing rules and regulations of the College, Hospital & Hostel as framed from time to time.
16. Students admitted to the College are expected to maintain discipline and professional dignity of high order and strictly adhere to the rules and regulations of the institute.
17. All the original testimonials submitted at the time of admission will be retained by the College until the successful completion of the course. The same will be returned to candidate after producing no dues certificates to the College office at the end of course.
